-Medical History-

Do you suffer from any ongoing medical conditions such as heart conditions, diabetes, asthma or other long-term physical disabilities, diseases or afflictions?

No medical complications.

Do you have any allergies to foods, inhalants or medications? Please list allergies and age of onset below.

Codeine, causes itching. Onset at approximate age 15.

Does your family have a history of any ongoing medical conditions such as heart conditions, diabetes, asthma or other long-term physical disabilities, diseases or afflictions?

Unknown.

-Psychological History-

Do you, or have you ever suffered from a mental affliction, or do you feel that you have problems with your state of mind? Examples could be anxiety, depression, seeing or hearing things that are not there, suffering from uncontrollable impulses, or entertaining damaging thoughts such as wanting to harm/kill oneself or do something damaging to another person. Have you ever abused controlled substances, alcohol, or prescription medications?

Bridgette suffers from a debilitation case of disorganised schizophrenia which causes her to suffer delusions of many sorts. She exhibits inappropriate emotional responses to stimulae, and unpredictable mood swings. She can be violent at random, particularly if threatened, but sometimes with no seeming provocation. Her behaviour can range from catatonic to child-like or completely coherent from time to time. The onset of this affliction is not know. The patient background is not known.

Have you ever seen someone for these problems or addictions? Or have you been hospitalised or housed in an institution or home for people who suffer from mental afflictions, or admitted to a drug rehabilitation centre? If so, for how long, and what was your diagnosis?

Bridgette has been with us for about ten years now. She has been in my care since admission to the facility. Her diagnosis is disorganise schizophrenia.

-Personal History-

Where were you born, and where did you grow up? Were there any complications during your birth? What was your infancy like?

Unknown. Bridgette came to the facility in a catatonic state, and no information about her past has been able to be collected. She was remanded to our care by the legal services of the State of Vermont.

Did you ever have any long-term separation from your parents?

Unknown. No family has come forth to identify themselves since admission.

Do you have any siblings? If so, did you get along well with them?

Unknown.

Did you attend public or private school, and if so, what were your grades like and how did you feel about it?

Unknown. Bridgette is capable of high-school level academics when she is in a coherent state.


Do you have an interest in the opposite or same sex, and if so, when did you become aware of this interest?

Bridgette shows little interest in sexual or emotional relationships with either gender.

Were you ever admitted to a psychiatric or correctional facility for juveniles, and if so why were you admitted, and how did you feel about it?

Unknown. No records have been found for a Bridgette Cavendish, as she was identified by Burlington police.

-Criminal History-

Have you ever been convicted of a crime? If so, what was the conviction, and was was the sentence served, or fine paid?

Unknown. No criminal record has been found for the name Bridgette Cavendish.

Have you ever taken or abused controlled substances? Have you ever had an addiction to prescription medications, or have you abused alcohol?

Unknown. Toxicology reports were clean upon admission.

-Personal Interview-

What are your goals in life?

Bridgette has little foresight past a few weeks, and thus little in the way of real life goals.

How do you hope that our facilities might help you in achieving those goals?

We hope that in our care, Bridgette will eventually progress to the point that she may one day leave psychiatric care, through the proper application of therapies designed for her.

Finally, tell us a bit about yourself.

Bridgette came to us at age eighteen, through The State of Vermont legal systems. She had been picked up by Burlington police late on an evening in October 2000. She was carrying the ID for Bridgette Cavendish, from which we assigned a name and date of birth. She has been at our facility in Vermont since then.

You hereby waive your right to informed medical consent before medication changes or medical procedures. You hereby agree to waive your right to hold Crane Pharmaceuticals and its Subsidiaries responsible for any personal harm or distress incurred during treatment.     All patients must attend group and individual therapy with their primary therapist.
