    I'm about 5'6 and and one hundred 36 pounds at last count. I'm black, I also have an appendectomy scar form when it was removed at age 11, I'm quite unremarkable.



-Medical History-

Do you suffer from any ongoing medical conditions such as heart conditions, diabetes, asthma or other long-term physical disabilities, diseases or afflictions?

None.

Do you have any allergies to foods, inhalants or medications? Please list allergies and age of onset below.

Not at all.

Does your family have a history of any ongoing medical conditions such as heart conditions, diabetes, asthma or other long-term physical disabilities, diseases or afflictions?

My older brother Ethan has Asthma and Lactose intolerance runs in my family, both of my parents and my eldest sister Rebecca are afflicted. Does that count?

-Psychological History-

Do you, or have you ever suffered from a mental affliction, or do you feel that you have problems with your state of mind? Examples could be anxiety, depression, seeing or hearing things that are not there, suffering from uncontrollable impulses, or entertaining damaging thoughts such as wanting to harm/kill oneself or do something damaging to another person. Have you ever abused controlled substances, alcohol, or prescription medications?

18 years ago I had a long bout with depression when my then fiancé was murdered by the first adolescent sociopath I have ever encountered. It was the start of a long depression for me. while the worst of it cleared up about 2 years after his death I was still depressed fro a good 4 year after. I've been told I also have a temper, but normally after someone does something to raise my ire.

Have you ever seen someone for these problems or addictions? Or have you been hospitalised or housed in an institution or home for people who suffer from mental afflictions, or admitted to a drug rehabilitation centre? If so, for how long, and what was your diagnosis?

I saw a therapist, a Dr. Emily Carter for my depression for a good three years, then I dealt with it myself.

-Personal History-

Where were you born, and where did you grow up? Were there any complications during your birth? What was your infancy like?

Born in Winston-Salem and raised in Charlotte, North Carolina. I was an easy birth; my mother's third child her body had probably adjusted. My infancy was normal, I didn't talk much until I was almost two years old but that might have, again, been because I already had older siblings willing to talk for me.

Did you ever have any long-term separation from your parents?

Daddy left for about 2 months once looking for a new job after he left his old one. I might have been 5 at the time. It must have been traumatic, I was very much a daddy's girl...

Do you have any siblings? If so, did you get along well with them?

I have 2 siblings, both older; my brother Ethan and sister Rebecca. The three of us have always been close; at time I'm slower to my sister. Ethan was a boy and sometimes wasn't kind when he wanted his little sister to leave him and his friends alone to so they could go off and do great masculine works. For a few years I hated him. Didn't help that occasionally Momma seemed to favor him. We patched things up when he came home form college decades ago. and he has all my nieces so we have to get along.

Did you attend public or private school, and if so, what were your grades like and how did you feel about it?

Public elementary and then a private middle school and the North Carolina School of Science and Mathematics for highschool. My grades were always middle to high, I never saw a C until college; it was a bit of a shock then but I managed. I always felt like I wanted to be the best, or at last my best in school. Slacking academically was not acceptable; if you could not accept in a subject it was accepted to a degree but refusing to perform academically was probably up there with serial murdering in my parents eyes, especially Daddy.

Do you have an interest in the opposite or same sex, and if so, when did you become aware of this interest?

Heterosexual, perhaps age 10; my best friend's grandmother worked for his family as a maid. I though he was the cutest thing. then my older sister's best friend Amelia was...stunning. I think I had a crush on her, but I was maybe 12 and I remember wanting to be like her, not really having a romantic angle to my adoration.

Were you ever admitted to a psychiatric or correctional facility for juveniles, and if so why were you admitted, and how did you feel about it?

Not applicable.

-Criminal History-

Have you ever been convicted of a crime? If so, what was the conviction, and was was the sentence served, or fine paid?

Not applicable.

Have you ever taken or abused controlled substances? Have you ever had an addiction to prescription medications, or have you abused alcohol?

I don't like alcohol, furthermore my body doesn't process it well. I've never had an addiction to prescription drugs and haven't even indulged in marijuana, let alone harder illegal substances.

-Personal Interview-

What are your goals in life?

To rip this application to shreds and go back home as I planned.

How do you hope that our facilities might help you in achieving those goals?

By letting me rip up the application and allowing me to return hope as I originally planned of course. Maybe a cup of tea, but I'd rather just go.

Finally, tell us a bit about yourself.

I do not belong in this place at all, and am probably writing this under duress and what I can only assume to be a heavy narcotic induced stupor courtesy of Dr. Gunner Bjorgen.
